SINGLE POINT OF ACCESS (SPOA) PROCESS

PLEASE READ AND DETACH BEFORE SUBMITTING A REFERRAL

In accordance with the NYS Office of Mental Health Single Point of Accountability/Access (SPOA) Initiative, all referrals for Supportive Case Management (SCM), Intensive Case Management (ICM), and Assertive Community Treatment Team (ACT) must be received, processed, and assigned in a coordinated and consistent manner at the local level.

Oswego County’s Adult SPOA process is coordinated by the:

Oswego County has four levels of service within the continuum of community based case management and care. From lowest to highest level of service intensity, they are:

  • Local Case Management
  • Supportive Case Management
  • Intensive Case Management
  • Assertive Community Treatment

The MHSC, under the general supervision of the Director of Community Services (DCS), decides what level of service the referred person needs and will receive. The process to arrive at this decision includes conducting a face to face intake with the referred person to explain the services and obtain their perspective on needs and desire to participate, collecting information from the referral source, and reviewing records. The MHSC will also review eligible referrals with provider program supervisors from Oswego County, Catholic Charities, and Oswego Hospital Behavioral Services to facilitate assignment and disposition. A peer professional from the local Peer Advocacy Program will also participate in the review. The final decision regarding a disposition is made by the Supervisor of the Assigned Program and the MHSC.

To request that someone currently enrolled in a program be transferred to a different level of service, please contact the current program to discuss the concerns. The program will then initiate a transfer review/ request with the MHSC. A person with an appropriate need for a transfer may have to wait until the new program has an opening. The MHSC would monitor and coordinate this process as well.

If a referred person or referral source disagrees with a decision made as a result of the SPOA process, please contact the MHSC to discuss the decision. If needed, a joint meeting with provider program supervisors and the DCS may also be arranged for further review of the decision to address the concerns.
