Transition planning and services are required by Federal and State law and regulation. It is a process which requires planning for your teen’s future. Advance planning for adult disability and/or mental health services and coordination of those services should begin before your youth exits school. Coordination of transition services will help your teen prepare for his/her future dreams and goals as an independent adult.
